A 6-week-old baby (Eddie) is brought to the Emergency Department (ED) by his mum and dad. The parents report that a week ago Eddie started vomiting after feeds. This has progressively worsened, and now he is vomiting after every feed. The vomiting often seems large volume and can be projectile, but afterwards he still seems hungry.
Eddie now has half the number of wet nappies as usual and is sleepier than usual. Nobody else in the family has been unwell, and he is exclusively breast-fed.
On examination, Eddie is sleepy. His nappy is dry. His observations are as follows: pulse rate 156, respiratory rate 40, oxygen saturation 100%, capillary refill time <2 secs.
