Your first patient is a 9-year-old boy with onset of left testicular pain over the last 2 hours. He is sore but feels otherwise systemically well. You note a blue discolouration on the skin in the region of the upper pole of the testes.
What is the most likely diagnosis and your management plan?

Your second patient is a 16-year-old boy accompanied by his mother. He complains of a sore testicle.
His mum says he first took ill about 2 weeks ago. He had fever, muscle pains, headache and swollen cheeks. She thought he had the flu. He was unwell for about a week then got better.
2 days after he seemingly recovered, he developed pain in his right testicle. On examination the testicle is swollen and tender.
