Your department receives a pre-alert from the ambulance service stating that they are bringing an unknown male approximately 20-30 years of age who has been involved in a fight and sustained a head injury. There were reports that he was initially unconscious, but he is now severely agitated and tried to attack the ambulance crew. He is now requiring multiple police officers to assist his transfer to the hospital. It is unclear whether his behaviour is due to some kind of substance use or due to a potential intracranial injury, but they are concerned that he will be hard to manage on arrival to the Emergency Department (ED) and are requesting a resus bay.
On arrival, there are four policemen restraining the patient on an ambulance trolley, who continues to fight them whilst shouting incoherently, trying to bite the officers as they hold him. The ambulance crew were unable to get any observations or properly assess him for any injury, other than the obvious bruising around his right eye and blood to the back of his head from a likely laceration caused in the fall.
