A 14-year-old male was bitten by an insect, whilst walking in the countryside. The insect bite was to his left upper eyelid.

The following day his left-sided eyelids became red, swollen and painful. He was seen by his GP and given a course of oral Flucloxacillin.

3-days later, he presents to your Emergency Department (ED), due to worsening left eye pain.

He reports double vision, which resolves when he closes his right eye. He feels the vision in the left eye is normal. His left eye appears to be bulging and is tender on palpation. He has difficulty opening his left eye fully and difficulty when looking around.
