A Hypertensive Emergency

A 56-year-old man presents to the Emergency Department (ED) with chest pain. It was fairly abrupt in onset whilst he was digging in the garden and was associated with a brief episode of collapse.

On examination he is pale and sweaty and is in considerable pain. Pulse is 110, BP 190/100, SaO2 92% on air, and RR 20.
