A Real Pain in the Neck

A 67-year-old man, with a background of type II diabetes, presents with pain in the left side of his neck, lower back pain and numbness in his left arm.

He had attended 1 week previously with haematuria having sustained penile trauma during a gym class. He underwent a cystoscopy following this.

On examination, he is febrile with a temperature of 38.6 degrees, heart rate 76 bpm, BP 125/67 and he has grade 4/5 power in the left arm (5/5 right arm) and reduced sensation to light touch on the left side.
