Burning Up

An 89-year-old lady is brought in by ambulance with hyperthermia and reduced sensorium. Handover from the ambulance crew is that she was found by her neighbour in the back garden sleeping with a book in her hand. Temperature with the crew was high.

Past Medical History includes heart failure, atrial fibrillation, and hypertension.

On examination, she is tachycardic, has a normal blood pressure and respiratory rate, oxygen sats 94% on 15 litres oxygen via non-rebreathe mask, and a temperature of 41.5oC. She is rigoring and unresponsive. Pupils bilaterally reactive. Her lungs sound clear, abdomen is soft and nontender.
