A 63-year-old diabetic woman had a dual chamber pacemaker implanted 9 years ago for symptomatic intermittent complete heart block. She is on life-long Warfarin following recurrent pulmonary emboli. Her follow-up in the Pacing Clinic has been unremarkable, but 3 months ago she underwent an elective box change for device end of life. Since that time the site of the implant has been persistently mildly uncomfortable. Two weeks previously she was treated by her GP for a possible chest infection with 5 days of Amoxicillin. The mild discomfort has persisted, but has become slightly worse over the last 2 days, although she has remained well in herself. She has attended the ED on a Saturday afternoon as she is concerned about the change in her discomfort.
