Complicated Chest Pain

A 71-year-old female presents to the Emergency Department (ED) acutely unwell, breathless and complaining of sudden-onset, severe central chest pain radiating to her back.

She is a heavy smoker and has a history of poorly controlled high blood pressure.

Her observations are: BP 165/105, HR: 98 bpm, O2 Sats 96% on 15L of oxygen, Temperature 36.5. 

An ECG is obtained, which shows sinus tachycardia with no other abnormalities.

A chest X-ray is performed:

A point of care abdominal ultrasound was performed and the following images were obtained
