Fishing for a diagnosis

A 51-year-old male attends your ED complaining of a sore throat and fever for 1 week. He also had diarrhoea and vomiting, and assumed he had the flu. These symptoms seemed to resolve but he then became more unwell. He then started to become short of breath, had a cough and develop bilateral leg weakness, pins and needles, neck pain and leg aches. He also complains of a severe headache. You note a small cut to his forehead which he acquired after a fall whilst fishing 10 days ago.

Past Medical History: Gout, Pancreatitis, Moderate alcohol consumption
