A 50-year-old man is brought to the Emergency Department (ED) complaining of persistent vertigo, visual hallucinations and floaters in his eyes. He is a known hypertensive and non-compliant with treatment. He has recently been diagnosed with grade 4 hypertensive retinopathy.
Observations: Temperature: 37.5, Respiratory rate: 16/min with oxygen saturations of 95% on room air, Pulse: 65beats/min, NBP: 260/150mmHg (similar in both arms).
Examination by fundoscopy showed flame-shaped haemorrhages, cotton wool spots, and optic disc oedema. The rest of his systemic examination was normal.
