A 36-year old business man has been having frequent palpitations, which have been spontaneously resolving, over the past month. He tells you that he has been too busy at work to seek medical attention.

Since starting a stressful new job last month, he has gradually increased his caffeine intake during the day time and increased his alcohol intake during week nights.

On Friday evening he is brought by ambulance to your emergency department (ED) clutching his chest and complaining that he feels unwell. His 12-lead ECG shows a supraventricular tachycardia. Intravenous access has been secured by the paramedics.
