A 56-year-old female presents with central crushing chest pain consistent with acute coronary syndrome. PR 105, BP 160/90; she is grey and clammy.

The paramedics have given her 10mg morphine IV, 10mg metoclopramide IV, two sprays of GTN and aspirin 300mg orally. She has subsequently vomited. Her verbal pain score is 7.
