A 45-year-old woman presents to the emergency department (ED) with a history of headache persisting for the past 4-5 days, accompanied by an inability to open her left eye for the past day. She does not report any vomiting, fever, confusion, seizures or loss of consciousness. Despite taking analgesics over the past four days, she has experienced only partial relief from her symptoms. Concerned by the recent onset of left eyelid drooping, she sought medical attention at the hospital.
On examination of her left eye, she has a dilated left pupil and ptosis, and the left eye is deviated downward and outward. Examination of the right eye and neurological exam are both normal.
