A 35-year-old male presents to the Emergency Department complaining of pain to the medial aspect of his right ankle following a twisting injury to the lower leg after getting his foot trapped between 2 rocks while climbing.
On examination he has a grossly swollen and tender medial malleolus with pain elicited on movements in all directions. He is mildly tender over the proximal fibula and is neuro-vascularly intact distally.
His x-rays are shown below.
