The Shocked Neonate – SAQ

A 22-year-old lady delivers a 36-week-old baby unexpectedly at home. The baby was initially pink with an Apgar of 9, and a birth weight of 7lbs.

While on route to hospital the baby suddenly becomes cyanosed and clammy so is brought to the ED Resuscitation room for review.

On examination she is centrally and peripherally cyanosed despite high flow oxygen, has a weak cry, heart rate 190, barely palpable femoral pulses, cap refill of 8 seconds, saturations 82%, respiratory rate of 60, blood pressure un-recordable.
