A 30-year-old male presents to the emergency department (ED) after waking up with shortness of breath and palpitations. He denies chest pain and syncope but appears pale.

His vital signs are as follows:

  • Heart rate: 212 bpm
  • Blood pressure: 112/72 mmHg
  • Oxygen saturation: 100%
  • Respiratory rate: 23 breaths per minute
  • Temperature: 35.2 C

On examination, his chest is clear with no raised JVP.
