A 70-year-old man with hypertension and dementia presents with central chest pain and exertional dyspnea for one month. He has attended to Emergency Department 2 times in the last month due to chest pain and discharged each time without follow up.
Vital signs are as follows: Temperature 37oC, BP 180/90 mmHg, HR- 80bpm, SpO2 97% on room air.
On examination you notice a systolic murmur that radiates into the carotids. The patient has ongoing mild chest pain and breathlessness despite aspirin and glyceryl trinitrate spray.
